All conference fees include continental breakfast, materials and continuing education credits. The registration fee is refundable (less $35.00) if your cancellation is received by 5:00 pm, May 28, 2008. Cancellations received after this date are non-refundable.
Registration Discounts are given to groups of 5 of more participants registering from the same agency. Offer valid only for Regular and Late Registration fees. Not applicable to Family PACT registration rates.
Group registrations can be submitted by mail or fax. Registrations for all group members must be received in the same envelope or faxed in together. If faxing, list all group members on the cover sheet. Groups may not be formed after individual registrations are received. Subtract the discount amount from the registration fees on the registration form.
