Written Material Format
Whether the written material is an Activity Book, Booklet, Bookmark, Card, Consent Form, Flip Chart, Info Sheet, Magazine, Medical Form, Packet, Pamphlet, Photonovella, or Slide Card.
Curriculum Format
Whether the curriculum includes Activities, Audiovisuals, a Bibliography, Case Studies, Charts/Graphs/Tables, a Course Outline, an Evaluation, Exam Questions, a Glossary, Handouts, Lesson Plans, a Pre/Post Test, Professional Resources, Protocols, Statistics, and/or a Workbook.
Book Format
Whether the book format is a Binder, Booklet, Hard Cover, Paperback, Spiral, or Velo Binding.
Audiovisual Format
Whether the audiovisual format is 16mm, Audio Cassette, Audio CD, CD-ROM, Computer Software, DVD, Filmstrip, Slide, or VHS.
Length
Number of pages, panels, or minutes that a material has.
Language
The language of the material. If the material is available and has been evaluated in another language, it will have a separate record for that evaluation. The ID numbers of these additional records are noted in the Evaluator's Comments.
Level of Comprehension
For video resources, this field is assessed by the evaluator based on their perception of the level at which a viewer would have to understand in order to follow the video. Choices are Elementary, Junior High, Senior High, and College.

These grade levels do not necessarily correspond to the level of schooling the viewer completed, but rather on the level at which he or she currently processes information (e.g. someone may have completed high school but process information at the junior high level).
Print/Font Size
Small < 10 point font in Arial or Times New Roman typeface.
Medium = 10-11 point font in Arial or Times New Roman typeface.
Large = 12+ point font in Arial or Times New Roman typeface.
Reading Level
This field and Reading Grade Level are two of the most important fields in an evaluation. It is estimated that over half of the U.S. population reads at the 5th grade level or below. Thus, if agencies want their health education materials to be understood, those materials should be at the 5th grade reading level or lower.

Reading level is assessed for English language materials using the well-known Fry Graph, developed by Edward Fry at Rutgers University in 1968. The Fry Graph is also adapted for Spanish readability based on the recommendations of a group of scholars at the University of Houston in 1980. Reading level choices are Elementary, Junior High, Senior High, and College.

Illustrations/Graphics
Whether the material has Comics, Line Drawings, Photos, or None.
Intended Audience
The evaluator's perception of the Intended Audience as Adult, Child, Disabled, GLBT, Parent, Pre-Teen (10-13), Professional, Senior Citizen, and/or Teen (14-19).
Gender Audience
Whether the target Gender Audience is Male, Female, or Male/Female.
Ethnic Audience
Whether the target Ethnic Audience as depicted in the material is American Indian/Alaska Native, Asian, Black/African American, General Audience, Hispanic/Latino, Multicultural, White or Other.
Contents
For books or curricula, this is the table of contents, or the major headings from the table of contents, providing additional details on what is covered by the material.
Evaluator's Comments
These comments represent the subjective reactions and opinions of the professional evaluator. The comments describe the content of the material and raise any concerns or criticisms of the material as well as naming its strengths.
Availability to Borrow
Available to Borrow or Not Available to Borrow by health care professionals from CFHC Resource Center lending library.
Status
States whether this material is available for purchase as of the last website database update. Written materials, posters, books, and curricula are either In Print or Out of Print. Audiovisuals are either Available or No Longer Available.
